Chapel began her medical career aboard the U.S.S. Enterprise, sacrificing a biology career with several university degrees in research medicine. Her fiancé, exobiologist Dr. Roger Korby, went missing in 2261. She joined Starfleet hoping that one day they would find him.

In 2259, she was a part of the Stanford Morehouse Epigenetic Project, and was assigned to the U.S.S. Enterprise under Dr. M'Benga as part of a civilian exchange. The project was involved in blind studies of pre-warp alien civilizations, and used genetic modifications to disguise the scientists to appear as aliens.

She became Head Nurse to U.S.S. Enterprise Chief Medical Officer Leonard McCoy in 2266.

In 2266, the U.S.S. Enterprise travelled to Exo III, the last known location of Roger Korby. Korby discovered technology left behind by an ancient civilization that could create android duplicates of humanoids. He planned on introducing the duplicates into key Federation positions. The crew of the U.S.S. Enterprise discovered Korby was injured shortly after arriving on Exo III, and this was an android duplicate. After realizing he was not real, the android destroyed himself.

Following the U.S.S. Enterprise's five-year mission of exploration, Chapel finished her own medical degree, and was promoted to Chief Medical Officer of the refitted U.S.S. Enterprise, now under the command of Captain Willard Decker. However, after Dr. McCoy returned to the ship in 2273 during the V'Ger crisis, she later willingly stepped down to allow McCoy to return as Chief Medical Officer.
